Who Can Make a Referral?
We welcome referrals from:
- Individuals who need care and support at home
- Family members, friends, or neighbours acting on behalf of a loved one
- General Practitioners (GPs) and practice nurses
- Social workers and care coordinators from the City of West Midlands Council
- Hospital discharge teams at local NHS hospitals
- District nurses, community matrons, and community health teams
- Occupational therapists, physiotherapists, and speech and language therapists
- Mental health teams and community psychiatric nurses
- Solicitors, advocates, and other professional representatives
- Charities and voluntary sector organisations

How to Make a Referral
By Telephone
Call us on 0333 772 4801. Our team will take your call, gather initial information, and arrange the next steps. If we are unable to answer immediately, please leave a message with your name, telephone number, and a brief description of the care need, and we will return your call as soon as possible, usually within two hours during working days.
By Email
Send your referral to info@olivecrosshealthcare.co.uk. Please include as much detail as possible, including the individual's name, date of birth, address, a summary of their care needs, any relevant medical history, and the urgency of the referral.

In Person or by Post
You are welcome to visit our office at OliveCross Healthcare Ltd, 3rd and 4th Floors, 84 Salop Street, Wolverhampton, WV3 0SR., or send a referral by post to the same address.

What Happens After You Refer
- We acknowledge every referral within 24 hours (usually much sooner)
- We contact the individual or their representative to introduce ourselves and arrange an initial conversation
- We schedule a comprehensive home assessment at a convenient time
- Following the assessment, we develop a personalised care plan in consultation with all relevant parties
- Care delivery begins, typically within 48 to 72 hours of the assessment
For urgent referrals, including hospital discharges, safeguarding situations, or rapidly deteriorating care needs, we can often arrange care within 24 hours. Please indicate the urgency when making your referral.
Information That Helps Us
The more information you can provide at the point of referral, the faster we can act. Helpful information includes:
- Full name, date of birth, and home address of the individual
- Contact details for the individual and their next of kin or main family contact
- A summary of care needs and the type of support required
- Relevant medical conditions, diagnoses, and current medications
- Details of any existing care or support services currently in place
- GP name and surgery address
- Funding source: local authority funded, NHS funded, privately funded, or to be determined
- Any specific preferences regarding care worker gender, language, or cultural background

Please do not delay your referral because you do not have all of this information. We can work together to gather what is needed once the process is underway.
